The image looks like someone squishing a bug. I don't think someone squishing a bug is meant to be literal.
As far as what it means - I think we all have to make our own interpretation. Obviously it means something to the creator of the image, and I hope he or she feels better now that they have gotten the secret off their chest. I think for alot of these images, what the creator means may be different than what we get out of them.

i can see that now or more correctly i saw it but before i could edit my post my paid for internet connection ran out. the first few times i saw it it reminded me of and ausie childrens book called the day my bum went wild/crazy.
