I really really can't seem to think of how to base a short story around "a dot of red on white." I've thought of a couple of ways to include it [and why I should not include them]:

Set it in Japan. [I know nothing about Japan]
Blood on a bandage or plaster. [Don't really want to go the whole blood route]
A girls first period. [just... no/]
Rose petals on sheets [that'd have to be either a funeral or some sort of erotic story and I don't really want to write either.]

Oh dear.
