John LeCompt is credited to have written this song. According to LeCompt, the original name of the song was C Sharp, the guitar tuning of the song. Originally, it was meant to be on Fallen, but never made the cut. An alternate look at the song describes John LeCompt’s secret addiction to Eggo waffles.

Eggo Waffles!? WTF!? (By the way, how/where do get the stories from? It's so damn interesting what inspired the artist to write a certain song and the whole story behind it is... well... I find it interesting...)

Waffles? What the.... So does this mean that he is in love with waffles and when he tried to get off his addiction it caused him pain? That sounds like something everyone could relate with (ah, what would life be without video games). Next thing you know when I try to stop playing video games I will end up writing a song about it lol. Still though, interesting story behind the song.

On a couple of websites I've seen this song being called "Forever You" and not "Forever Gone, Forever You". So what's the right title of it?

it's "Forever Gone, Forever You" and to your second question, I think earlier... not sure... almost positive, though...

I found this video and I'm not sure if it's real because the bits with videos from other Ev songs have some TV Network branding on them, but the girl with the black wings looks so much like Amy, are those parts of an official ev video?

It's just a fan video. They used clips from various music videos and shows of Evanescence. The girl with the wings is Amy from Broken music video (Seether feat. Amy Lee).
